School Counselor - Rachel Jave

My name is Rachel Prince and I am the licensed school counselor at Transition Plus Services.  This is my 12th year of being a licensed school counselor and in the Minneapolis School District. It is a great honor to be a part of the Transition Plus Services community and I look forward to a great school year!

School Counseling Services

The counseling program at Transition Plus strives to align with the American School Counselor Association (ASCA) model for a data driven, comprehensive school counseling program.  Licensed school counselors work with students in the social-emotional, college/career, and academic domains.  School counselors provide students with support through a variety of means including individual work with students and their families, small counseling groups, and counselor taught classroom lessons. School counselors also collaborate with families, teachers and other staff to support students.     

The licensed school counselor at Transition Plus supports students in the following areas (and more!):
